Permanent disability: Man loses legs during workplace incident

The worker subsequently became caught in the conveyor, and emergency responders were forced to amputate his legs to free him.

Investigating officials assert that prior to allowing workers to perform maintenance on the machine, the company should have taken the necessary precautions to lock it out and prevent it from being turned on. The company has reportedly been fined just over $283,000 following the incident due to safety and health violations.
